Post-Incident Reviews

Post-incident reviews (PIR) capture timeline, impact, root cause, and action items after significant incidents.

Requires the Post-incident reviews module (see Permissions).

  1. Open Post-incident reviews (/post-incident-reviews) and click New.

  2. Link the related incident and fill in summary, timeline, and impact.

  3. Document root cause and corrective actions; assign owners and due dates.

  4. Share the PIR with stakeholders; update action items until closed.

  5. Mark the PIR complete when all actions are done or formally accepted as risk.
